Hooked up shackle wrong, did no coordination with other driver, didnt have comms, didnt stop things when he got concerned just let dude pull again, let other guy do a 4 or 5 meter full speed pull, multiple strong pulls on kinetic with no rest, cheap amazon shackle, no weight blanket on the rope, no use of recovery boards to get traction out with 1 pull...
... any single one of these being fixed would have stopped this failure chain cold.
This reads like every USAF accident reports ever, a string of mistakes and ignoring warning signs.
